I've been DMing for years, there are many humanoid enemies with 3 attacks that don't dual wield or use bonus actions for them, they are included as their attack action. Much of this is due to the wonky nature of the CR system. A gladiator has 15 HD and gets three attacks per round because of it and is only considered CR 5. That being said, the 10 HD bandit captain is a CR 2 encounter so it wouldn't be a surprise to see them up against a lower level party. For example, a bandit captain has 10 HD has gets two on-hand and one off-hand attacks per round.
